04. The Architecture of Resilience
A tree loses its smaller branches first. Under drought or cold, it does not choose which limbs to sacrifice; the outermost, most delicate growth simply stops getting enough water and sugar to sustain itself, and it withers. The trunk survives. The main boughs survive. But the fine lattice of twigs that once caught the most sunlight is gone, and the tree that remains is a smaller, thinner version of what it was, still alive, still recognizable, but with less surface area for catching light.
Something structurally similar happens inside a brain under chronic stress, and the discovery of it has quietly overturned one of the most familiar public stories about depression.
For decades, the popular account of depression was chemical: a shortage of serotonin, a "chemical imbalance" to be topped back up with medication, the way a low tire gets air. That story is now under serious strain. A 2022 systematic review led by Joanna Moncrieff at University College London examined six separate lines of evidence — serotonin and its breakdown product in blood and spinal fluid, the activity and density of serotonin receptors and transporters, studies that depleted tryptophan (the raw material for serotonin) to see if mood collapsed, and genetic studies of the serotonin transporter gene. Across all six, the review found no consistent evidence that depression is caused by lowered serotonin activity or concentration. The paper was downloaded more than a million times and covered widely in the press. It was also, predictably, contested: other researchers, in formal replies published the following year in the journal Molecular Psychiatry, argued that the low-serotonin idea was never really psychiatry's working theory to begin with, and that the review's methods overreached. The dispute is real and ongoing. What is not seriously disputed is that the low-serotonin story, whatever its scientific status, is no longer where the field's most active researchers are looking for the mechanism of depression.
Where they are looking instead is at the physical shape of neurons — the branches, not the chemical bath the branches sit in.

A neuron is not a smooth wire. It grows a tangle of thin extensions called dendrites, and along those dendrites it grows tiny nubs called spines — this is where one neuron receives a signal from another, the actual point of contact, the synapse. More spines means more connections means a denser, richer web of communication between brain regions. Fewer spines means a thinner web. In 2005, a researcher named John Radley and colleagues subjected rats to repeated restraint stress — a standard, humane laboratory model of chronic psychological stress — and then examined the neurons in a brain region called the medial prefrontal cortex, an area heavily involved in mood regulation and decision-making. The stressed rats' neurons had lost 16 percent of their apical dendritic spine density and 20 percent of their total apical dendritic length, compared to unstressed controls. The branches had thinned. The twigs were gone.
This was not an isolated finding. It sits inside a much larger body of work associated with Ronald Duman at Yale, one of the primary architects of what is now called the neuroplasticity, or synaptic, hypothesis of depression. The core claim is a reframing: depression is not fundamentally a deficiency of a single chemical messenger, but a structural loss of connectivity — literally fewer physical links between neurons, concentrated in the prefrontal cortex and the hippocampus, the brain's memory-and-context hub. The same stress that shrinks the tree's canopy shrinks the neuron's branch structure. And the same treatments that help depression — chronic antidepressant use, in the animal studies — measurably reverse it, restoring dendritic length and spine density in these regions.
The causal direction is not just correlational speculation. A 2019 study led by Conor Liston at Weill Cornell, working with Duman as a senior collaborator across a multi-institution collaboration spanning Weill Cornell, Stanford, and the University of Tokyo, gave the connectivity story its most direct test yet. Mice exposed to three weeks of a stress hormone lost dendritic spines throughout several prefrontal subregions, exactly as the structural model predicted. When those same mice were later treated and their new spine growth was tracked, the new connections proved to be the thing standing between recovery and relapse — a mechanism explored more fully in the next chapter, where the timing of that regrowth turns out to matter as much as its existence.
There is a molecular signal that appears to drive this shrinkage and its reversal, and it goes by an unglamorous acronym: BDNF, brain-derived neurotrophic factor. Think of it as a kind of fertilizer for neurons — a protein that encourages dendrites to grow, spines to form, and existing connections to survive rather than be pruned away. Across a large and consistent body of rodent research going back to the 1990s, chronic stress lowers BDNF in the hippocampus and prefrontal cortex, and chronic antidepressant treatment raises it back up. This finding is not seriously disputed at the level of brain tissue in animals; the falling and rising of BDNF tracks the shrinking and regrowing of the dendrites almost like a dial and a gauge wired to the same circuit.
The honest limit of the evidence
Here is where the story has to slow down and be exact, because the neat animal picture does not translate cleanly into a claim about the living human brain — and a serious account of the science has to say so plainly, in the same breath as the promise.
No one can safely open a living human skull and count dendritic spines. Everything known about spine loss and BDNF depletion inside an actual human brain, during an actual episode of depression, is inferred — from brain imaging, from postmortem tissue studies of people who died with a history of depression, and from one imperfect proxy: BDNF measured in blood.
Blood BDNF is not the same substance, in the same place, doing the same job, as BDNF inside hippocampal tissue. It is a stand-in, and by 2022 several independent teams had tried to find out how good a stand-in it is. The results were not encouraging. A 2022 meta-analysis by Tiwari and colleagues did find that peripheral, blood-measured BDNF was significantly lower in depressed patients than in controls — but with substantial inconsistency from one study to the next, the kind of noise that makes a single number hard to trust. A separate 2022 systematic review looking specifically at treatment-resistant depression found that blood BDNF levels did rise significantly with treatment overall, but that rise showed no association with whether a patient's depressive symptoms actually improved. Stranger still, the same review found that non-invasive brain stimulation — a treatment with real clinical benefit — did not raise peripheral BDNF at all. The blood signal and the clinical outcome were moving independently of each other, like two dials that looked connected but were not.
This is not a minor footnote. It means that a blood test for BDNF, however tempting as a simple biological marker of depression or recovery, cannot currently do that job. The animal-tissue story — stress shrinks the branches, BDNF is the fertilizer, treatment restores both — remains the leading account because the mechanism is coherent, replicated across many labs, and consistent with what is separately known about the prefrontal cortex and hippocampus in mood regulation. But the bridge from a rat's brain tissue to a person's blood draw is narrower and shakier than it is sometimes made to sound.
What makes this account matter, despite its incompleteness, is what it implies about depression's nature. A chemical-imbalance model suggests a substance is missing and needs replacing, like a nutrient deficiency. A structural-loss model suggests something closer to what happens to that tree in drought: a living system that has pulled back its own reach, thinned its own connections, in response to a sustained hostile environment — and, critically, a system whose branches can in principle grow back. The next two chapters follow two very different attempts to make that regrowth happen fast.
Overturned if: a well-powered study using a validated, tissue-correlated biomarker showed that peripheral BDNF changes reliably track both depression severity and antidepressant response across independent patient cohorts — closing the gap between the animal-tissue finding and a measurable human signal, and turning the current cautionary note into confirmed evidence.
